EIN - employee identification number : local unit completes SS4 form and sends to KY PTA office. The IRS issues this number which is to be used to open the local PTA bank account. This number remains with the PTA as long as the unit exists. Generally all PTAs in KY have numbers that begin with 61-. All PTAs have their own EIN number.
5110 - this is the group IRS number that identifies the individual PTA as a member of the KY PTA. This number is used on federal tax forms when it asks for the group number.
A-14300 - this number is the Kentucky state sales tax exemption number. All PTA units in Kentucky use this number to purchase items tax free. PTAs should never use the school's tax number.
PTA I.D. number - this number is located on the label of any mailing the local unit president receives from the National PTA. It is a eight digit number located in the upper left hand corner of the label. This number must be used when ordering materials from the National PTA catalogue and filling out some forms from NPTA. Each PTA has its own number. A sample is below.
