-This is an eight digit number that can be found on your letter of good standing from Kentucky PTA and on the mailing label from all mailings you receive from National PTA.

-Name of your PTA/PTSA
Issued/Expired
-The date the card is issued and this card is valid for one year
Signature
-This is the signature of the PTA member
